No surname start with Y in a Slavic country. The English Y is a J sound so the surname Yurcik would be JURCIK.
Jerry might have been Jiri in old country.
Did find JURC^IK, JURIC^EK, JURIC^KO and JURKA in Czech Republic's phone book.
