Robert,

The Form 66-1 is not a complete document. The block numbering goes from #19 to #31 and appears to be the second page. Do you have the other portion of the document that would be numbered #1 thru #18 and would have his identifying information and a possible unit of assignment?

Having this piece to the puzzle would be a great benefit. Having an incomplete Form 66-1 that doesn't show his identification whould be difficut to push for an award justification. Also, you may have noticed that the award information was added after the fact of signing the document. The dates of the awards are after the date of signing and the last line for the Silver Star is the only line in all caps and isn't alligned with the lines of the document as the others are.
