I know that the Waupaca Public Library has the (complete?) survey on microfilm, with typed index. I've used it before for Waushara cemeteries. The folks there are very helpful, and I suspect that a letter or phone call to the Library Director will get you some results. Probably a small fee. May also be available at a library in Waushara County, or at the State Historical Society in Madison. Also maybe at the Regional Archives at the University of Wisconsin-Stevens Point. Good luck!
