I have been viewing the microfilm of the Newport Pagnell Parish Register at my local LDS Family History Center.
I found the baptism of "Steven Waller son of Doctor John Waller baptized November the 24th" 1676.I cannot say whether or not he was a medical doctor, but here is a definite primary source to verify that at least in 1676 her was referred to as "Doctor."
I found all the other baptism of his children reported in the IGI, beginning with Leonard Waller who was baptized on October 21, 1670.No other baptism reference used the description "Doctor" or the abbreviation "Dr."He is referred to as "Mr. John Waller" in the baptism record of his son William Waller, baptized on September 24, 1671.I also found the burial of "Mary, wife of Mr. John Waller" on May 14, 1716.He if referred to as simply "John" Waller in all the other baptism and burial records regarding his children.
