I have located at least one copy of the Stanley Vivian book albeit at a somewhat hefty price. It is being offered by a out-of-print book dealer in Los Angeles who in turn is evidently brokering it for a third party. The asking price is $100 USD. The book is described as a third printing of a private publication with glossy paper covers, 193 pages.
