What I read is he reported to post Nov. 4th the in other column is the ditto marks on his line from above state
"Surrendered under GO 26 (General Order 26) ?? cs attached to..." then his line continues D 2nd Cavy, surrendered himself on 24th of NOV as Frank Dunbar Co. B 3rd Arty and Co. c 12th Inf, alias as James Adams of 'K' C, 7 Inf the Jan 1874 roll he is listed as under "Casualty of Post*" and same information as above in remarks.

So looks like he used an alias James Adams best I can see.
Have you tried Fold3 website, I believe it has Military databases and is in partnership with the LDS FamilySearch.org website.
