Podkamieniu ad Rohatyn

I found a post card in my grandfathers prayer book with the following caption:

Oltaez Weilki w kosciele parafjalnym w Podkamieniu ad Rohatyn

it has a very old picture of the inside of church showing the alter and behind it a very elaborate and tall series of paintings including a large madona and child, statues and a round starburst at the top.

Can anyone make the translation? or perhaps be familiar with the church?

I have a second document from another family member. It is a copy of a tesimony of baptism.

Respublica Polonia
Ducatus: Stanislavicuis
Districtus: Rohatyn
Dieciesis: Leopolicusis
Decanatus: Strilieusis ??
Parochia: Podkamincia
at the bottom of the page..
Testimonium ortus et baptismi L. skt. 162. Drukarnia Ch. Szkolnika w Rohatynie.

I can't make out the date other than it is 23. IV. 1920 something......

Any guidance would be much appreciated.
