The institution was the Royal Naval Hospital. Howard was the Inspector General of Hospitals.
It also states married 17 years , 3 children born all still living. So as there are just 2 daughters with him maybe George is his son working elswhere in the hospital. On the other hnd it doesn't fit with length of marriage if George is 21 !!!
