The problem with the source list report going haywire is a known problem in version 3.0, the solution to which can be found in the Knowledge base. Basically, the first time you print a source list report, you won't get any errors. However, once you print or preview it, note that there are programming codes in the Selected field of the Source List report window. If you attempt to print or preview the report with those codes appearing, you'll get errors.
What you need to do is clear those codes by doing one of two things. Either click the Clear All button and then reselect the items you want to print, or select one more item and then clear that one item. When you do this, the Selected field will have the correct names and not the programming codes and you won't get the errors when you print.
As for getting duplicates, the solution to that is to select the "Library" option rather than the "Project" option. When you choose Project, the source will print for each time it is used in the project. When you choose Library, the source will print just once.
