Well first name and middle name fit, full date of death and maiden name. Is this your Ruth? Keep in mind accuracy on death certificates only as accurate as the knowledge of the informant.

Several people are working on this line. The first one shows among the children Ruthie Maude, birth date, but no death date. I would contact some of these people, find our the sources for their information and share what you know on your branch.
