Can only answer a few of your questions- Thank you though for providing at least the background.

William Harrison Agnew died - Jan 27, 1930 Reagan Co - (4691 certificate #)

No other information from here-

Phema b Dec 21, 1881 Cokesburg, PA-
married Oct 30, 1916 Hominy Ridge, PA
moved to Texas June 10, 1926 (?) settled in Caddo lived for 3 years/ Lived in Texon {Reagan Co} until 1931 when she moved to Abilene.
Two children preceded her in death:
William Robert (Billy) Agnew
Mrs. Tully A. Mayer

She was survived by two brothers -Max and Lawrence Smathers of Leeper, PA-
Four sisters:
Mrs Hugh Croasum - Springfield, OH
Mrs. Sid Croasum of Youngstown, OH
Mrs. Myron Buhl of Cane, PA
Mrs. Homer Neal of Dundee, N.Y.

Three daughters:
Beatrice A. Shackleford of 4049 Avondale (Abilene)
Mrs. Grayson Smith of Gig Harbor, WA
Mrs. Jerry Canatsey of Brentwood, MO
John Burr Agnew, Sr. of Eden

The type on the paper is really blotchy and it is hard to make out the dates but read them the best I could.
Burial was to be at: Fairmount Cemetery, San Angelo, Tom Green Co., TX

She was living with the daughter in town at the time of her death
Checked the Texas death records online at rootsweb but could not locate William 'Billy' in them - nor was he in the 1903-1940 microfilm so I would assume he died between 1930 and 1964 but did not have time to check the microfilm (each 2 year period is on a different roll - takes awhile).

Don't know the first name of the daughter that died so can not narrow that down either.
