Joan is correct. Don't waste your money on a request for death certificate until you know exactly what name she used. The same is true if you havent verified the date of birth you now show. If it is somehow wrong it's doubtful they will "search" for the dob. Also, don't go to the state for verification of marriage. Go to the County Clerk, Collin County. You can probably get a photocopy of the page from the 'marriage book' for $1.00 plus postage as opposed to the State's $9.00 "just for verification". You'll be time and money ahead if you deal with the County Clerk on both, the marriage and the death certificate.
