A water boy was person designated to take a group's canteens to a water source, fill them up and return them. There was no official designation for a "water boy"...it was simply whoever in the squad volunteered, or was volunteered, to fill this duty. Medical corpsman may also fill this duty around the hospitals.
All that said...no it was not unusual for someone in their 20's to have this job since the average age of a soldier in the Spanish American War was 27.6 years of age.
