Actually, sadly, I quickly replied to you last night and in my tiredness never realised this was your mom. Sorry about that!!
Okay, the only thing I can think is that her parents were still living in South Africa at the time of her death, as shown on the death notice. It also says she left a will. If your grandparents were in her will, it might have have been necessary to get it in the South African records that she had passed away.
Or a more straightforward answer might be that she probably had a South African passport as well and that it would have been law to have her notated in the population register as "deceased".
As for it being in the Free State records - if you look at the stamp on the death notice, it says "Master of the Supreme Court", Bloemfontein. Now, I am not sure if you know, but Bloemfontein is the judicial capital of South Africa. So I assume, seeing that she passed away in London, the death notice would have been filed in the judicial capital, as there would not have been a relevant local court to have filed it with and therefore it now shows in the Free State archive.
I am not sure about the relation. My married surname is Webb. We are both South Africans, but my husband's family originally come from Northampton and were shoe makers.
