A common misconception, no US official at port of Ellis Island, Baltimore, Boston, Philadelphia changed anything. The manifests were written up at the port of departure from the document your ancestor provided.
Do you know if he was naturalized citizen? Did he have social security numbers? http://ssdi.rootsweb.com Did he have to fill out a draft registration form? These and few others US documents should have his place of birth and his "original" surname.
