I can't imagine what the 5 was for....it sure isn't part of a Quaker date which consists of a month, day and year....I have many family members buried in Middletown burying ground, and have posted on the web, an earlier transcription of burials there, although I don't believe it contains anything as late as 1916. The Middletown meeting started out "life" in Chester Co, in the late 1600s, shortly the Penn's arrival in 1682, but is now in Delaware Co, not Bucks....unless there is another Middletown meeting house.
