In general the father's name is used. In formal circumstances and on may official records you will see the fathers name first followed by the mothers maiden name (eg. Rivera y Garcia). Rivera would be the fathers last name and Garcia would be the mothers maiden name. When I was in the military many Puerto Rican servicemen were wearing name tags with both names displayed. This is common practice in Spain and Latin American countries that maintain Spanish traditions. Hope that answers your question.
