in Russian- dominated Poland, now WielkiePolska, once Kalisz
province/wojwodzwu, also Kalisz powiat/county. Your document
is a Russian/Cyrillic birth cert./ Akt urodzenia is Polish!
"Wypis z duplikatu.."is "written duplicate from civil recs".
I've forgotten most of my Cyrillic but the words "Roman Kath
olic parish from the year 1894" directs you to LDS/Mormon's
"search place" on "family search's Library Catalog showing 4
RC church group filmings, 28, 30, 8, and most importantly,
11 rolls with # 1923052, item 1 covering births/urodz. 1842-
1844, AND, 1846-1904..NOTE the latter dates!! The church is
Sw/St. Mikol~aja/Nilolaus, and the records are in LATIN, not
Cyrillic. GO GIRL!! The surname SL~OWIK, "SWOH-vik", means
"word" in Polish, so.."I've given you the word!""POWODZENIA,
Pani!!!"
