You have to make your own templates. On the TOOLS Menu, select "Preferences," and then "Templates." You can add individual fields (or subtract them) by clicking "Modify," and then using the > to add or the < to subtract fields. Once you've got all the fields you want, then you can click on any particular field and drag it up or down to the position you want. Once you're done, give your new template a name, and then select it as the default template.
