In your PAF4 folder there are two files for your data file with extension names .paa and .pax, hence, if your file is Test.paf, then delete Test.paa and Test.pax. These are index files. PAF4 will re-build them which will correct any errors in them.

Upgrade to PAF 4.0.3, too. This will eliminate that bug from happening again.
