I just put the name of the cemetery on the same line as the town, county, state; e.g..
Bemus Point Cem., Ellery, Chautauqua Co., N.Y.
Kanosh City Cem., Kanosh, Millard Co., Utah

I do the same thing for churches (christenings, marriages), e.g.
St. Botolph's, Bradenham, Buckinghamshire
First Baptist Church, Taunton, Bristol Co., Mass.
