Transposition of Notes between 2 different RIN's

I have repeatedly entered a note for one individual, RIN 5704, a William Newman, and hit save. I can exit out and shut down my PAF (5.2.18). When I go back to RIN 5704, the note is there.
But, when, I then go to RIN 35, a William Shaw, the last two letters of his existing note have been deleted and the note I had just entered for RIN 5704 has been transposed to the end of the note for RIN 35.
When I go back to RIN 5704, his note is gone, and the last two letters from RIN 35 is there.
(I always back up, and daily save a version of my file on my D drive. In checking this D drive today, I saw that a note I have added to a 3rd person, was deleted and was added to the note for RIN35.)
I then clean up the two notes, and the problem repeats.
After each of about 6-8 tries, the problem is the same.
I am a long time user and have never seen this before.
I am not cutting and pasting.
Any suggestions would be most appreciated.
