Location of Jerusalem Lutheran & Reformed Church

I am trying to determine the geographic location (as precisely as possible) of the church known as the Jerusalem Lutheran and Reformed Church of Western Salisbury (see, for example, H. Ray Haas, 1911). One reference gives it as "located at Macungie in the drainage of Cedar Creek and the Little Lehigh"). The location is relevant to genealogical research I'm conducting. Any help will be much appreciated. Thank you.
