The PA Department of Health would probably be a good place to start to find out what happened to the hospital records, but don't be surprised if they deny you access to them citing patient confidentiality even long after the person is deceased and even if you are directly descended from the person.

As I said they could have the death certificate but not able to find it. This website about the grassroots effort to get the older Pennsylvania state death certificates online may interest you: http://users.rcn.com/timarg/PaHR-Access.htm . If we succeed it allow you to look for it yourself rather than depending on a clerks quick glance through the records.
