Richard Otis(2) was at least seventeen years older than than Susanna. There is a strong possibility that he had a wife prior to his marriage with Susanna. This is speculated by Noyes, Libby& Davis in GDMNH. It also speaks to the points you raise about her age when John was born, and the ten year gap before the next child.
GDMNH gives John, the son of Richard(2)as being the one taken to Canada and since known as John Baptiste. Stephen Otis had two sons Stephen and Nathaniel who were taken to Canada and became known as Joseph Marie and Paul, GDMNH does not list a son John for this Stephen.
Stackpoe, in "History of New Hampshire" says Mary Pitman
the wife of Stephen Otis, was likely taken into captivity with her children John, Steven and Rose. He makes no mention of Nathaniel. Incidentally GDMNH makes no mention of Rose.
All historians and genealogists make mistakes, I have found errors in both sources that I have mentioned. Having said that, both are excellent works, with GDMNH being (in my opinion) the best source of information in my library.
