Yes, normally there would be a section for that information; however, it depends on what the informant knew. Many times the information is not on the form because the person giving the information on the deceased just did not know.

You can Google "Oregon Death Registration Began" and find out when the state began requiring filing of death certificates. Prior to that date, sometimes a certificate would be filed with the appropriate county.
