It has been a few years ago so i do not remember the address. I did a google lookup and used the address that was listed on the net. As I recall i was sent the records and billed for copy fees. The records include the admittance report, his kids name, parenst names, wifes name birth information his habits, his medical records. They are very informative. If your request gets to the hospital they will find the right department I think.
