Jerry, apparently you have to go to the Tulsa place IN PERSON. No mail service for death certificates. The place is the Tulsa City/County Health Dept. at 3rd & Utica in Tulsa. The building in on the east side of Utica Avenue and can be seen from the 3rd & Utica intersection.It is a two story bulding. Just walk in, go straight and then turn right. You will need the date of death. If you have to get the certificate from Oklahoma City, I can send you a form to use. Will need your mailing address.
