I received a zerox copy of a number of records, some are hard to read, but with some work I'm sure I can clear them up. There was a copy of the OK.Standard Certificate of Death, a couple of admission records, a Physician's Certificate, a Sheet with a Diagnosis, and a report on her condition when she was admitted. There is sheet with a little Family history, but the handwriting is very, very hard to read. I would love to see the miciofilm or even the original records. They send you a form to fill out and get notarized and mail back to them and they deal with every thing else. You have to tell the Judge why you want the information,I just said tracing my Family Tree. And the only charge was for the copies. Hope this helps.
