Thank you so much for responding. Could you tell me what "type" of information you were able to obtain? I keep thinking of them because they are a resource, but haven't know what type of information they might provide.

The ancestor that died there is actually the brother to my gr grandfather----the ancestor who was in the hospital never married or had any children, so there are no direct descedants for him, but do they require that you are a direct descendant?

One last question---do you have to take the form to a judge, or mail it to a judge, or do they take care of that?

I just know nothing about any of this, so it really helps to have the direction and advice from someone who has "jumped through the hoops" already. I really appreciate you taking the time to help me.
