Check John Steve Kozar's WWI draft registration card at https://familysearch.org/search/collection/1968530

There are 4 letters on the line above Austria Hungary that may be his birthplace. It appears that he may not have signed the card as the signature looks like the same handwriting as in other info the card.

His death cert is online at https://familysearch.org and I found his death notice in the Youngstown newspaper but the right edge of the notice was chopped off when the paper was microfilmed. I put a - to indicate the missing letters.

"Youngstown Vindicator", May 5, 1932, 1-19-8:
KOZAR-John, 52 Palmer Ave., Ca-bell, died at St. Elizabeth's Hosp- Wednesday, 1:45 p.m. aged 56. He - in Czechoslovania. Member of St. -thias Lodge, F. K. S. U. No. 200. -rvived by wife, Maria, 4 sons, J-, Steve, Michael and Joe; 8 daught- Catherine, Anna, Mary Elizabeth, Z- and Helen, Member of St. Elizabe- Church. Funeral Saturday, nine a. - from St. Elizabeth's Church. Interm- Calvary Cemetery, officiated by - Joseph Kos-.
