Thank you for the response. His body was not buried in Prescott, AZ. His remains were transfered to his hometown and he was buried at Woodside Cemetery, Middletown Ohio, Section 25/GR, C367.

Thank You for the information. From it, I was able to go to the Yavapai County Arizona web site and find the copy of the death certificate you had referred to.
