In June I sent and received a copy of my fathers birth certificate, also born in Toledo, Lucas County in 1902. There was no real "birth certificate" just a writing in a large log book, but they sent me a certified copy of a birth certificate. I sent a money order (no checks accepted) for $5.00 to: Lucas County Probate Court, 700 Adams Street, Suite 200, Toledo, Ohio 43624. I sent a stamped self addressed envelope along with the note telling the name of the person, birth date, parents, and place of birth. I only took about two to three weeks to get a reply. Good luck.
