Do you have anything, by any chance, of a Sarah Ann Johnson? She was born in Ohio about 1817, married a John Mercer COOK in Knox County (possibly on 6 Aug 1834, where county records indicate a J.M. Cook married a Sarah Johnston) and eventually may have moved to Richardson County, Nebraska, after 1860. She died about 1864, probably either in Richardson or Knox.
