Union Chapel in Morris twp- help please with address

Does anyone know the address for the Union Chapel Cemetery in Morris twp.? Might be called Morris Union Chapel.

I need to mail them a request for a lookup that is not listed in the book Cemetery Records of Knox County, Ohio, Vol. 1 for Morris-Union Chapel.
Death Certificate say's:
MARTHA SELINA/Celina [Poole]FOGLE
b.11-9-1829 TollGate,Ritchie Co.,WV
d.4-8-1911 in Mt.Vernon, Knox Co.,OH
Date of Burial: 4-11-1911
Place of Burial or Removal:Union Chapel

The only info I can get is:
Morris-Union Chapel {North of Hyatt Rd (TR 399) 1 mile east of Old Mansfield Rd (CR 5) Non Sectarian
Really need an address!!!
