I don't know why son John's says his mother's maiden name was Jelek. Obviously, Ann was mistaken. Daughter Dorothy's Death Certificate is a bit messed up. It shows Dorothy Pesek, passed July 19, 1930. Date of birth, 1912. Father, John Pesek, born in Bohemia. Mother, Elezbeth Kotapis/Katopis, born in Cleveland. Informant was her mom & she signed it "Elezbeth Pesek". It appears Dorothy had heart failure. Id#: 0745133 Name: Studer, Edward E.
