Have you checked to see if it is still in existance? That would be my first step, if I were you. Any white-pages on the internet could find a phone #. I'd also do a check using the search engine (be sure you put the subject in " " makes a big difference. I'd try
"put the church name here" ohio
as the subject, and variations of that idea. Also, I'd check around to see if there is a national headquarters for the denomination (I'm sure there is), and contact them to see if there are any extant records.
