advice on how to contact

I recently found out the name of someone that may be my father. I also have located an address and phone number for him. I want to contact him but am not sure the best way to do that. I don't know how to approach him about myself. He has no idea that I may be his daughter. I also recently found out who my birth mother is and she is that one that gave me his name but as it turns out he was never even told that she was pregnant so I will be a complete shock to him. I don't want to blow this opportunity to find out about him but I don't want to scare him off by taking the wrong approach. Does anyone have any advice on what I should say to this man? And would it be better to write a letter or to actually make the phone call? HELP!! ( :
