The "O" prefix indicates "from" as in descended from. In Wales, the prefix was "ap," in Germany "Von," in Holland "Van," and in Scotland "Mac."
The "O" prefix clearly marked an Irishman as being of Gaelic origin, and hence secondary citizens. Many families, including mine, dropped the "O" to anglicize their surname.
Since Ireland gained its independence, many whose ancestors had dropped the "O" have since added it back to their name.
