If you go to www.health.state.ny.us/nysdoh/vr/forms/genealogy.htm
you will find forms for birth, marriage and death certificates. Considering your ancestor died after 1880, you can mail the form with $11; the death certificate will name his parents, their birthplace, and the decedant's birthplace.
