Hi Robert,
I'm convinced the mistake lies at NYC Vital Records but obviously I cannot prove it. I've had various searches done there including one that only gave date of death and burial as they are the two things that must be certain. I included the name only in the additional information box as well as age, address, etc to make cross-referencing easier. I know earlier indexes are filed chronologically so it shouldn't be too much of a leap for a staff member to match a death/burial date to the extra info I gave - NO LUCK!
It's interesting you mentioned the probate as it was quite a complicated process - the executor being required to locate 2 next of kins who were finally accepted as presumed dead so maybe the certificate wasn't filed until after this was resolved - I'll check when this was exactly - I'm not clued up on the legal side of things so I can only guess? Don't think this will throw much up though, as when I've checked the indexes there doesn't appear to be any Mary O'Neil's remotely close to the info I have from Calvary and a few years either side of 1916!
