Michael, I'm certain there must be a death certificate. By law, there would have to be! Her will could not have been probated without her death certificate. Here's what I think the problem probably is, and it is a frustrating problem that one does encounter: The people later tasked with indexing the death certificates often had difficulty deciphering the handwriting. So my feeling is that Mary O'Neil's surname is seriously misspelt in the New York City death index.
