Request a certified death certificate from the New York City Department of Health. http://www.nyc.gov/html/doh/html/vr/vrdappl.shtml The daughter of Carl Earl Hornbach will need to display her birth certificate in order to obtain this death certificate in accordance with regulations given on the link. Genealogical copies of death certificates are available fifty years after that death registration. As Carl Earl Hornbach died in 1965, a genealogical copy can be requested in 2015. Examine the death certificate after receipt to determine burial location. If not indicated, inquire about a burial transmittal permit which is required to transport cadavers outside the vital records office district. That permit indicates burial place, but the permit is not issued if the burial occurred within the district. Notify Administrator about this message?
