I just looked at 2 death certs from the Department of Health of the City of Brooklyn. One was from 1884; the other was from 1886. Neither of them have either parents' names nor a blank space to enter that information.
It does ask for birthplace of mother and father.
After consolidation in 1898, the certs were issued by the City of New York and I believe they did ask for parents' names. They are definitely there by 1906. As usual, that information was only as good as the knowledge of the informant.
