Lori,

Send a check for $4, made out to STATE REGISTRAR, and mail it to State Registrar - Search Unit, NJ Department of Health and Senior Services, P. O. Box 370, Trenton, NJ 08625-0370.

Include in your letter of request the following: Name of the deceased, number of copies requested (one), place of death (city, town, township, county), residence if different from place of death, age at death, father's name and mother's maiden name. The last two are not always necessary, but you should provide it if you can. Also include your name and address, your telephone number, and your relationship.
