state hospital entry on 1920 census (question)

I found a person on the 1920 census for merrimack co NH who at first I thought was a patient in the state hospital but then noticed the last column that is filled in on the page shows occupations. It has him as attendant while others are nurse, clerk, etc. Why would the census taker do the census of the work location and not the home location? Did folks live in resident at the hospital? I'm confused. Pam
