St. Joseph's Church, Lakeport or Laconia?

Hello:
I am looking for some information, my Grandmother (Annette LaVertue Hooper) was killed in a car accident in 1940 in Laconia, she was pregnant at the time and they were unable to save the baby.
According to the newspaper clipping that I have she and the baby girl were buried together.
The funeral mass was at St. Joseph's Church, Is that Lakeport or Laconia? Does St. Joseph's have it's own Cemetary?
Also wondering about A.W. Simoneau Funeral Home, does it still exist??
Where would I be able to get death records or burial records??
