When you go to the Bureau of Vital Statistics, there is a form that they will request that you fill out. On this form, you fill in as much info as you have. There will also be the question asked; "How are you related to the deceased?" (on death certificates) or; "If this is not your record, how are you related?" (on birth records) As long as you are related, they aren't too hard to get along with. They are a little more strict on the birth records, as they may possibly be living. I'll give you a clue, if you aren't 100% definate of a date or a place, put "approximately" along with the date or place. That way the search won't be confined to just a specific date or place. Should you want the forms ahead of your going there, I can mail you a copy if you contact me directly at my email address. The Bureau of Vital Statistics is only open during the week, whereas the State Historical Society is open on weekends.
