You want a Form NATF 85. The online page where you go to request that forms be mailed to you is:

It takes a week or two to get the form in the mail. There is a place on the form where you can put the record number 21584. This speeds up the process A LOT, because they know exactly which record to pull. If there is a photocopy of the pension record index card (sometimes you can find these on Ancestry), it helps if you attach it to the application (my cousin phoned them in Washington and asked them about this). It costs $37.00 for the long version.There are only military pensions for Abner and Stephen MAYNARD, not Silas MAYNARD.
