John, I found the form G-1041 A for UCIS and will submit it, hoping it will list address of last residence and his wife's name who had died in the old country prior to his coming to the U.S.
I posted about my great grandfather's Polish/Russian passport in the Polish forum. He was Joseph Dogovsky, last living in Seirijai having married his wife there. Their first child, my grandmother was born there as well. I have his Naturalization petition but it does not list his parents or address in Seirijai. I though maybe his Blue Book/passport would link to further info.

Hello. I have a translation of my great grandfather's Blue Book/Passport: given to Yosel Volphovich Dogovsky on June 14, 1903 by or in Suvilytsky province. He served in the infantry of Omsk regiment #96 from 1899. Discharged August 2, 1902. Recruited by Seinsky district. Discharged pass certificate #117.
His birthdate is listed as Jan 1, 1877. All of the U.S. records I found on Ancestry have different birthdates and years, but I am sure they are all his records because of the U.S. address and his wife & children.
Questions:
Would the passport birthdate be accurate?
Can I use this passport to trace his family line - where he lived, his parents, etc? He is said to have been born in Warsaw, with parents from Warsaw. His 1903 U.S. ship manifest says last residence Lomsk, which is strange because he left his wife and child in Seirijai where they were married and all lived (they came over 3 years later).
